The credentialing process requires submission to the DAP of a completed application form, supporting documentation as identified on the application form, and payment of a processing fee. Please note, that until all documentation and payment has been received, the application will not be processed.

The credentialing process involves review and verification of information by both DAP staff as well as DAP physician reviewers who are in active practice. For this reason, the credentialing process can take up to 2 months from the date that the application has been determined as “complete” and ready for review. Applicants are encouraged to familiarize themselves with the credentialing requirements and application process to prevent delays due to incomplete documentation.
